The Appointment includes the following:
Nutritional Assessment:
Your weight & height will be measured. Wheelchair scales available
if necessary.
Dietary Assessment:
A detailed diet history is taken (i.e. an account of your usual eating
pattern)
Dietary Advice:
an individualised meal plan is discussed with you.
Written Information:
A diet sheet is given to you, with all the information you need to help
you achieve your diet.
How To Make an Appointment
This Out-Patient Service is available to both Out-patients & In-patients.
We need a letter of referral from either your GP or Hospital Consultant.
An Appointment letter will then be sent to you.
